Terry informed me that on March 16, 2004, musictap.net posted another tidbit about Tori's upcoming DVD/CD:

We mentioned earlier that Epic has a Tori Amos DVD in store for May 4. How about a little sweetener to go with that news? Ok..Epic will also release a CD album of the same title, Welcome to Sunny Florida, also on the same day. It looks like it may be an EP and might accompany the DVD as a bonus but the details are kind of sketchy at the moment.

Also, Matt Chamberlain mentions the DVD on his web site. From this we see that Loren Haynes was involved with the making of the live DVD. (Thanks Brett.)
